Job Summary and Qualifications

The Manager Case Management will be a subject matter expert and facilitate process standardization to promote improved outcomes, maximize resource utilization, and achieve organizational goals. The Manager will support the strategic objectives of the Case Management department by coordinating and working with teams to complete priorities and initiatives in a timely manner. Supports the assessment, planning, coordination, and development of case management processes including areas of automation and information systems.

This position requires an individual who is a self-starter and team player, has the ability to manage multiple priorities, work with minimal supervision on assigned projects and activities, and demonstrates excellent communication and presentation skills. This individual must be able to adapt quickly to change and be able to collaborate with multiple teams.

Major Responsibilities: 


  • Provides support to Facility and Division leadership, and identifies areas of opportunity for continuous improvement including ongoing education and resources as needed

  • Including automation and information systems designed to promote corporate and division standardization, utilization, integration, and optimization

  • Supports the optimization of case management technology

  • In conjunction with the Director Case Management, is responsible for collecting, analyzing, and evaluating outcomes and operations data

  • Manages reports and metrics regarding program performance and performs root cause analysis

  • Uses the data from reports to facilitate the identification of gaps and support improvement opportunities

  • Effectively manages tasks and priorities in order to complete objectives timely

  • Creatively and innovatively contributes to the development and design of Case Management programs to incorporate current research and evidence based best practices in order to remain current with the industry

  • Collaborates with appropriate Case Management leaders to establish policies and procedures, evidence-based toolkits, education/training, communications and reporting capabilities to support and monitor programs successfully

  • Conducts ongoing research to remain current and knowledgeable in industry trends and best practices related to Case Management

  • Establishes and maintains effective and positive relationships which promote teamwork

  • Participates on assigned projects, committees or initiatives, promoting effective communication between entities and the department leadership

  • Practices and adheres to the “Code of Conduct” philosophy and “Mission and Value Statement”

  • Other duties as assigned

Education and Experience: 


  • Bachelor's degree required

  • Master's degree preferred

  • 4+ years of experience in acute care preferred

  • 3+ years of experience in Case Management required

  • Occasional Travel: The job may require travel from time- to-time, but not on a regular basis.

  • Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation, preferred
