Job Summary

Under the general supervision of the Senior Director of Well-being and Associate Director of Counseling and Psychological Services, the Clinical Case Manager will provide effective referrals, follow-up, continuity of care within the Student Wellness Center, between Counseling and Psychological Services, the Student Wellness Center, and other departments on campus. This individual will develop and maintain a current database of available service providers and assist referred students in making use of community resources.

 

Key Responsibilities

  • Coordinate and monitor referrals to support services, including referral and liaison services with housing agencies, social services, mental and physical health services, financial support agencies, food services, legal services, etc. for students in need
  • Develop and implement systems for follow-up with students to ensure continuity of care
  • Assist the clinical staff in arranging dispositions for students following assessment, crisis, initial consultations, and as clients are completing treatment at Counseling and Psychological Services
  • Work closely with psychiatrist(s) and physicians in ensuring continuity of care and arranging dispositions and referrals for high-risk students
  • Assist in psychiatric hospitalization of students and aftercare planning following hospitalization
  • Offer limited consultation to students, faculty, staff, parents, other healthcare providers, for purposes of referring students to community and private resources (food banks, shelters, etc.) as appropriate
  • Serve as a liaison to community and private mental health providers, social services, and hospitals
  • Develop and maintain a departmental database and website of referral resources and community services
  • Serve as a liaison and advocate for enrolled students, their families, and peers. Perform general student advocacy/case management duties including, but not limited to: managing student emergencies, performing faculty/staff/administrative consults, responding to parent and community inquiries
  • Act and advocate for the referred student as he/she navigates the academic, student life, and health care landscape by working with relevant on-campus and off-campus providers to ensure that student’s needs and accommodations are understood
  • Represent the Student Wellness Center at university functions that require trainings, presentations, or orientation

 

Knowledge, Skills & Abilities

  • Experience with crisis counseling and mental health interventions
  • Ability to plan, coordinate, and initiate critical actions necessary to implement administrative or group decisions and/or recommendations
  • Ability to carry out a variety of professionally complex assignments without detailed instructions
  • Strong oral and written communication skills. Must possess excellent customer service and public relations skills
  • Thorough knowledge of, or ability to learn CSU policies and ability to apply that knowledge to individual situations
  • Ability to interpret and explain policies to various staff members
  • Ability to analyze and define complex organizational, policy or procedural problems
  • General knowledge of University, procedures and practices related to student services and programs
  • Ability to compose and appropriately format correspondence and reports, knowledge of English grammar, business writing, punctuation, and spelling
  • Ability to carry out very complex assignments without detailed instructions
  • Ability to advise students individually or in groups on varied and complex matters
  • Demonstrated ability to effectively present information and respond to questions from various groups including managers, faculty, staff, students, media, and community members

 

Required Qualifications

  • Equivalent to graduation from a four-year college or university in a related field plus upper division or graduate coursework in counseling techniques, interviewing and conflict resolution where such are job related
  • Four years of progressively responsible professional student services work experience which includes experience in advising students individually and in groups, and in analysis and resolution of complex student services problems. A master’s degree in Counseling, Clinical Psychology, Social Work or a job‑related field may be substituted for one year of professional experience. A doctorate degree and the appropriate internship or clinical training in counseling, guidance or a job‑related field may be substituted for two years of the required professional experience for positions with a major responsibility for professional, personal or career counseling

 

Preferred Qualifications

  • Master’s Degree in social work and eligible to sit for licensure; OR Master’s Degree in psychology or counselor education
  • Experience with diverse student populations on college campuses
  • Experience working with college students, specifically student athletes and survivors of sexual assault
  • Strong working knowledge of local community resources
  • Strong knowledge of private health insurance benefits as well as Medical
  • Strong knowledge of case management services within a medical facility
  • Multilingual/multicultural
  • Proficient in database management and computer record-keeping systems

 

Compensation

Classification: Student Services Professional IV
Anticipated Hiring Range: $5,450/month - $6,100/month
CSU Salary Range: $5,351/month - $7,633/month
