Position Summary:
We have an exciting opportunity to join our team as a Program Counselor, Bilingual/*Older Adult Center* - Brooklyn.

In this role, the successful candidate The Program Counselor will be based at the Shore Hill Older Adult Center and be responsible for providing supportive services to older adults, including intake and assessment, case assistance, Information and referrals, friendly visiting, short-term counseling, and psychoeducational/support groups. The Counselor will be responsible for documenting service in the DFTA database (STARS/Peer Place), and for maintaining case logs.

Job Responsibilities:

    • Provide case management assistance on concrete services and benefits to seniors, including information, referral and advocacy.
    • Other duties as needed
    • Maintain professional standards in all aspects of work, including direct service, advocacy, tracking and follow-up, consultation, professional development, documentation and supervision.
    • May supervise interns and volunteers who assist with case assistance, I&R with center members
    • Will participate in trainings and in staff meetings
    • Collaborate closely with other colleagues within FHC and NYU to provide high quality services and programming to clients.
    • Collaborate with Shore Hill Older Adult Center and Services for Older Adults staff on programming and operation of the site
    • Document service in DFTA database (STARS/Peer Place) and maintain case logs
    • Provide friendly visiting to select isolated clients living in the community
    • Facilitate and conduct psychoeducational workshops on topics identified by seniors
    • Provide crisis intervention as needed,
    • Provide short-term supportive counseling to seniors

Minimum Qualifications:

  • To qualify you must have a B.A. in Social Work or related field
  • 1 + years experience working as a case worker/case manager
  • Strong interest in issues of aging and in serving older adults in an immigrant community
  • Bilingual in English and Russian, or Spanish, or Chinese (oral and writing)

Preferred Qualifications:

  • MSW from an accredited social work graduate program
