The purpose of the Clinical Social Worker I/II (CSW I/II) position is to engage with high utilizers of the San Francisco Sobering Center, with efforts to coordinate care, initiate services, and work to stabilize these individuals. This population largely has co-occurring substance use, mental health and medical conditions, frequently resulting in homelessness and disconnection from stable health services. The Citywide Case Management Program at the Sobering Center of the San Francisco Department of Public Health provides integrated substance abuse and assertive case management services to clients suffering from severe alcohol use disorder who are high users of the Sobering Center, ambulances, emergency rooms, hospitals, and/or detox services. The program is sited in and will work in seamless collaboration with the San Francisco Sobering Center to engage clients on the streets, in shelters, in SROs, in residential treatment facilities, in the county jail, or wherever clients might find themselves. The CSW I/II provides culturally appropriate, trauma-informed, evidence-based substance abuse services, case management, crisis intervention, treatment planning, individual and group counseling, and/or collateral/family services. The job will involve the ability to lift 50 lbs. and climb stairs.
