The Nurse Practitioner/Registered Nurse will be responsible for providing care and case management for patients and delivering clinical care to chronic kidney disease and end-stage kidney disease patients. This position will be working closely with complex renal patients in their home, dialysis center, by phone, virtually, and electronically as needed. This individual will work with patients and the care team through in-person, virtual, telephonic, and electronic means of communications, care coordination and management. This position will be the clinical leader for the field staff. They will work with the field-based clinical staff, non-clinical staff, and interact with local nephrology providers, hospitals, dialysis units and ancillary services.

Required Qualifications
Current license as Nurse Practitioner in California
Minimum 5 years as nephrology provider

COVID Requirements

As a part of this process, you will be required to provide information or documentation about the reason you cannot be vaccinated. If your request for an accommodation is not approved, then your employment may be terminated.
