The Social Worker at Martha Eliot Health Center will be responsible for:
  • Coordination of Boston Children’s Hospital Primary Care ACO flexible fund project with community based social service agencies addressing social determinants of health.
  • Assessment of families to determine eligibility for social support interventions, referral and follow up, ongoing collaboration with community agencies, and Primary Care social work.
  • Clinical documentation, data collection and organization of meetings.
  • Supervision of Resource Specialist staff.
 
To qualify, you must have:
  • Master's degree in Social Work, and LCSW in the state of MA. LICSW preferred.
  • Experience with social services agencies, understanding of social determinants of health and trauma highly preferred.
  • Analytical skills to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to solve problems OR design relatively complex systems and programs.
  • Communication skills in order to effectively deal with conflicting views or issues and mediate fair solutions, and well developed writing skills.
  • Previous staff supervision experience highly preferred.
  • Diverse candidates encouraged to apply, fluency in Spanish is preferred.
