This position will be working from home in the Greater Chicago area and will require local travel 25-50% of the time

The Kidney Member Advocate is an integral member of both the case management team and the nephrologist providing care coordination to support members/ beneficiaries receiving treatment for chronic kidney disease. The advocate is a culturally competent health care worker who collaborates with members, families, physicians, and the healthcare system to ensure chronic kidney disease member needs are appropriately and effectively addressed.

A primary function of the advocate is to establish and help maintain communication between members, their families, physicians and the health care system, as well as to increase a member'sengagement in their own care and adherence to their treatment regimen. The individual who holds this position exemplifies the CVS Health mission in accordance with CVS Health policies and procedures, including complying with all CVS Health Customer Service Standards.

Main Job Functions include:
- Maintain communication with members, families, dialysis units and the nephrologists to monitor member satisfaction with chronic kidney disease care experience.

- Cultivate interpersonal relationships and foster trust with members from chronic kidney disease (CKD) through end state renal disease (ESRD).

- Verify member appointments and follow-up with nephrologists to assure planned dialysis start

- Provide outreach for all program member and coordinate care and services including access management throughout the lifecycle.

- Provide support and education for member related to fluid management and sodium restrictions.

- Daily monitoring of inpatient discharges, ER discharges, and referrals.

- Monitor memberengagement in scheduled appointments for dialysis treatments, procedures, tests and consults.

- Work with care team to identify extrinsic barriers to adherence (i.e. transportation, financial concerns or pharmacy) and work with member and team to alleviate barriers

- Coordinate by phone with interdisciplinary care team to facilitate care coordination and ensure all member medical needs are met

Required Qualifications
- Previous experience in healthcare, social work, ambulatory health setting
- Strong written and verbal communication skills
- Exceptional interpersonal skills
- Strong ability to multi-task
- Proficiency with basic Microsoft Office programs required
- Ability to function independently, to organize, and be self-motivated

Preferred Qualifications
- Experience working with CKD, ESRD or members with multiple chronic medical conditions
- Bilingual in English/Spanish highly desirable

High school diploma or GED
