The Collaborative Care Therapist is responsible for providing general psychological care and treatment to patients in medical facilities such as the clinic, health center, or public health agency.

Primary duties may include, but are not limited to:

  • Performs mental status examinations, therapy, and preventive health measures.
  • Reviews psychological diagnostic tests to identify and assess patient’s clinical problems and health care needs.
  • Records physical findings, and formulates a plan and prognosis, based on the patient’s condition.
  • Discusses case with the Physician and other health professionals to prepare a comprehensive patient care plan.
  • Submits the health care plan and goals of individual patients for periodic review and evaluation by the Physician.
  • Administers or recommends individual therapy or other forms of treatment.
  • Refers patients to the Physician for consultation or specialized health resources for treatment.
  • This position will be primarily non-direct patient facing; you will be leading our roll out of the Collaborative Care Model across California, helping to provide expert psychiatric care to patients as delivered by their primary care clinicians
  • You will be the quarterback of the Collaborative Care team, serving as the single point of referral for all incoming BH requests, and utilizing your clinical judgement and expertise to triage the patient to the appropriate level of BH care using our internal CareMore resources
  • Approximately 20% of your time (flexible and subject to change) will be spent directly caring for patients

Qualifications

What you contribute?

  • Requires an MS in Social Work, MS in Marriage and Family Therapy, or a PhD in Psychology; 5 years of experience; or any combination of education and experience, which would provide an equivalent background.
  • Current license as an LCSW, LMFT, LPC, LMHC, LMSW, Licensed Psychologist etc. in the state of CA required.
  • Experience in working with a cross-functional, primarily digital care team preferred
  • Experience in working with the behavioral health population with cognitive disorders preferred
  • Experience working with the geriatric population preferred
  • Experience working with the institutionalized patient population (e.g., in assisted living facilities, skilled nursing facilities, etc.) preferred
  • Satisfactory completion of a Tuberculosis test is a requirement for this position.
