Regional Medical Center of San Jose is an acute care hospital with a Level II Trauma Center. The hospital provides a host of technologically advanced services including Cardiovascular, Neuro, Orthopedic, Bariatric and General Surgery, Interventional Pulmonology and services for Women. Regional holds Joint Commission advanced certification as a Comprehensive Stroke Center and a Get with the Guidelines- Stroke Gold Plus Performance Achievement Award from the American Heart Association/American Stroke Association. County-designated STEMI (heart attack) Receiving Center designated Lung Cancer Screening Center, a member of Extracorporeal Life Support Organization (ECMO) and certified by The Joint Commission in Sepsis care.

SCHEDULE: .001 FTE (PRN-Per Diem) / SHIFT: As Needed

POSITION SUMMARY: The Patient Placement Coordinator (PPC) coordinates the assignment of patient care beds within the hospital and works closely with teams to facilitate expediting the discharge process.  Empowered by Senior Executive Leadership, the PPC collaborates with facility departments and medical staff to promote appropriate, timely patient placement in beds and positions so that patient care is not delayed.  The PPC facilitates regularly scheduled bed management meetings with facility staff to identify and communicate bed utilizations issues.  The PPC will identity, trend, and report barriers to patient throughput.  The PPC will report directly to the ACNO.

 

 

 

 

 

 

Qualifications

EXPERIENCE REQUIREMENTS: 2+ years’ of relevant experience 

EDUCATION REQUIREMENTS:

Minimum:   Associated degree or High School Diploma 

Preferred:    BA/BS  

 

KNOWLEDGE/SKILLS:

Effective conflict resolution skills,

Ability to manage multiple tasks and priorities simultaneously

Effective strategic, logistical and critical thinking with pro-active decision making and negotiation skills

Ability to establish and maintain collaborative and effective working relationships

Ability to communicate effectively in oral and written formats

Able to conduct crucial conversations in confident manner

Able to work independently with minimal supervision and able to work effectively within teams

 

Proficient in MEDITECH and other applications/technology supporting bed management
