**JOB SUMMARY**

This job reports to the Director of Network Performance and manages a team of network performance managers and client liaisons in each micro-market to serve as the single point of accountability for provider facing PAC programs, initiatives, scorecard review, and quality and technology/vendor partnerships. The incumbent provides education and articulates the vision of HCS while working with facilities to improve overall quality of care in scorecard metrics. Manages, maintains, and grows current business and prospects new opportunities for HCS provider partnership. Supports all strategic and owned hospital providers and provides direct consulting support to build their post acute model. The incumbent helps to design and oversee territory alignment in each market. Supports all corporate relationships within post-acute in each micro-market and provides high level education to these markets. This job will facilitate regular meetings with Executives in the Post-Acute Space and with key strategic hospital executives to assess trends, keep abreast of escalating issues and analyze root causes to determine corrective action steps. The incumbent will train customers on core services and expand customer understanding and usage/adoption of key tools. Will also maintain and provide customer information on service trends, care initiatives, improvement opportunities, etc.

**ESSENTIAL RESPONSIBILITIES**

+ Perform management responsibilities including, but not limited to: involved in hiring decisions, coaching and development, rewards and recognition, performance management and staff productivity. Serve as a leader of teams, ensuring that people, processes and technology are aligned towards the achievement of team objectives. As a team leader, understand that success of the team is the result of the entire team’s output and that the program participant is evaluated in the context of individual results along with the sustained results of the team.

+ Manage corporate relationships with individual establishments/customers (i.e., Skilled Nursing Facility, Home Health Companies, LTACH, IPR, and Hospitals). Manage the preparation of business plans and target lists. Oversee the priority accounts in accordance with the market growth plan and volume shift goals. Establish effective internal and external working relationships and forge partnerships among internal and external stakeholders and network post-acute providers. Facilitate key meetings with health plan leadership to review results of network, vendors, and other PAC strategies.

+ Lead in the strategic implementation of key initiatives for Highmark Inc and HCS around vendor engagement and implementation; and lead in the strategic implementation of post-acute clinical transformation through network development and analysis of high performing partners. Provide best-in-class training services, organize training/education opportunities, and promote participation/inclusion with key accounts and partners.

+ Lead short and medium-term duration projects, building an action oriented skill set that enables successful project delivery and confidence from project team members and matrix partners.

**EDUCATION**

**Required**

+ Bachelor's Degree in Business Administration, Nursing, or related field

**Substitutions**

+ 6 years related and progressive experience in lieu of Bachelor degree

**Preferred**

+ Master's Degree in Business Administration or Nursing

**EXPERIENCE**

**Minimum**

+ 5 years of Healthcare Industry, or related industry experience
