Riverside Community Hospital, Founded in 1901, is a 478 licensed bed, full-service acute care hospital in the heart of the Inland Empire.  RCH has been recognized as a Top Performing Hospital and has invested in a new campus expansion project that includes a new 7 story patient tower, 3 story medical office building, and a recently completed 5 level state of the art parking garage.

POSITION SUMMARY

The Patient Placement Coordinator serves as a member of the patient care team whose primary responsibility is facilitating continuous, smooth, efficient patient flow of admissions, unit transfers, and early discharges. Coordinates with each department to ensure processes for admissions and discharges are carried out in an efficient manner identifying and resolving barriers to these processes. Gathers and reports patient flow data collection for admissions and discharges on occasion. Makes suggestions for improved patient flow processes. Participates in education and presentations to staff and management on patient flow barriers to admissions and discharges. Must have strong interpersonal skills for interfacing with team members, all levels of staff, physicians, patients, families and other contacts to facilitate patient flow activities. Must possess the ability to effectively function in a stressful environment. 
 
Reports To (title): Manager, Patient Care Services
 
SHIFT/SCHEDULE: Mixed-Night shift .9, 12 hour shifts with rotating weekends
 
 

Qualifications

POSITION REQUIREMENTS:

 
Experience: 
  • Previous patient flow or house supervisor experience preferred.
  • Minimum of 2 years acute hospital experience within past 5 years.
  • Also prefer some case management background.
Licensure/ Certification/ Registration: 
  • Licensed as a Registered Nurse in California. 
  • Current BCLS certification. 
  • Certification in nurse administration preferred 
Education: 
  • Associate or Bachelor Degree or Diploma in Nursing
