Job Summary

JOB SUMMARY
At Houston Methodist, the Population Health Advisor is responsible for understanding the programs and initatives being deployed by Houston Methodist Coordinated Care, identifying practice improvement opportunities and integrating clinical programs and resources into the practice workflows with the intent to optimize the patient experience, quality of care provided, and positively impact resource utilization.

Support the physician practices by analyzing performance reports and data to inform decision-making, process, and program implementation. Monitor and implement educational and process interventions based on practice-level data, trends and identified opportunities.

PRIMARY JOB RESPONSIBILITIES

PEOPLE - 25 %
1.Creation and maintenance of relationships with assigned primary care practice physicians and staff, high value specialists, urgent care centers, and other community stakeholders.
2.Advise primary care practices, physicians, nurses and other clinical staff to assist them on their conversion to value-based care.
3.Provide educational and training sessions related to Houston Methodist Coordinated Care programs and initatives.  These include topics related to quality, utilization, and patient satisfaction.

SERVICE - 25 %
1.Travel throughout the Houston, TX area to meet with physician practices to support clinical practice transformation.
2.Communication and support on payment methodology transition(s) e.g., education & review of compensation packages.

QUALITY/SAFETY - 20 %
1.Identification of process improvement opportunities in Houston Methodist Coordinated Care physician practices
2.Provide reporting on primary care practice quality metric performance. This includes the dissemeination of ¿gap closure¿ lists and best practices to promote continuous improvement.

FINANCE - 10 %
1.Dissemination of quality and efficiency reports to Houston Methodist Coordinated Care physician practices. 
2.Analyze reports for practice specific trends and provide guidance and ongoing support to primary care physicians and practice staff for continual process improvement.

GROWTH/INNOVATION - 20 %
1.Testing and refining of workflows that support sustainable transformation, and guide practices as they expand interventions to additional patient populations
2.Collect best practices related to quality and effieciency initateives and disseminate best practices to other practices and Population Health Adviosrs.

EDUCATION REQUIREMENTS
oBachelor¿s Degree
oMaster¿s Degree in applicable field preferred

EXPERIENCE REQUIREMENTS
oFive plus years of experience in practice management, provider relations and project management
